USYSA Risk Managment (PDF) - This form allows you, or your coach, to obtain a coaching pass.  They are good for two years.  You must turn in all completed forms to the club registrar to send in.  Do not send in the form directly to the NYSW office. 

You've Been Spotted Cards (PDF) - This is a simple invitation card that you can give to people in the community that directs them to our website.

Team Profiles Worksheet (Word Doc) - You need to have a team history for when it is time to register for tournaments.  This is a form that helps to organize the information you will eventually need. 

Medical Release Form (PDF) - Every player on your team needs to have a completed medical release form.  A copy should always be avaialbe to the coach or a designated parent whenever the team is together. 

Parent Contract (PDF) - Each parent in the club should be required to sign and turn in the contract.
